End-of-Life Planning
Advance directives are legal documents that provide guidance on the medical and/or mental health care you would choose for yourself in the case that you are unable to voice those wishes at a critical time in the future. They help to ensure that you will get the care that you want. And they help loved ones and caregivers know they are making the right choices about your care.
We provide Advance Directive support in a series of two meetings.
During the first, we will discuss your wishes and available options for medical intervention or renunciation in end-of-life care. A follow-up meeting is spent answering any questions, reviewing and recording the formalized plan, and discussing next steps, including how to inform loved ones and medical/care practitioners.

Advanced Directives
When we talk about “keeping” or “sitting vigil” in the context of end-of-life, we’re talking about how we are present with someone during their process of active dying and death. But how we envision our final time can be very different from how others see their ideal transition at end-of-life. Thus, preparing a Vigil Plan helps loved ones and caretakers understand our vision for our own Best Death.
We provide Vigil Planning support in a series of two meetings.
During the first, we will discuss your vision for Your Best Death. We will contemplate such things as:
Who you’d like to be present
How you’d like them to interact with you
What space you’d like to be in
What kind of lighting, music, or scents you’d prefer
Any desired spiritual supports
How you’d like to share your wishes (video?, letter?, phone calls?)
At our second session, we will revisit new thoughts and do a final recording of your Vigil Plan. We will help to ensure it is shared with those you wish to share it with.
**[Sitting vigil during active dying is not included in this package. Options for this specialized care can be found under Companioning & Vigil Services.]
